Notice of Collection of Personal Information The personal information collected on this form is collected under the authority of the Municipal Act, 2001 and the Day Nurseries Act, R.S.O. 1990, and will be collected and shared with all child care agencies selected by the applicant, for the purposes of placing your child in an available space. If your child has attended a child care centre in the past, information regarding outstanding child care fees may be shared between child care operators before your child is placed again.
